Information required:  Team or Sport: (IE:) Varsity Basketball, SHS JV Baseball, etc.

File Number: = Number located in the upperleft corner the Picture you wish to order.

Size of Print: = 4x6, 5x7, 20x30 etc.

Number of Prints: =   How many of each print.

Example:  SHS Hockey, 5218, 4x6 3ea. 5x7 1ea. 20x30 1ea.
